EvergreenHealth is honored to present the Ninth Annual Seasons of Hope Luncheon, benefiting EvergreenHealth’s Hospice and Palliative Care Programs. Join us in supporting this important luncheon, scheduled for Thursday, November 9, 2017 at the Lynnwood Convention Center, by becoming a Table Captain. The Seasons of Hope Luncheon is a great way to introduce and engage family, friends, colleagues and others with EvergreenHealth and the many programs that Hospice and Palliative Care have to offer.
We invite you to help us meet our goal by filling a table of ten. Our goal is to fill the Lynnwood Convention Center with 500 guests who are willing to support Hospice and Palliative Care services at Evergreen by making a financial contribution at the luncheon. Your participation is critical to our success.
Table Captains can participate in one of two ways:
- Purchasing a table of ten and filling it with your guests
- Serving as the table host by purchasing your own individual ticket and inviting nine to purchase seats at your table.
We promise to make it easy and rewarding for you and your guests.
To register as a Table Captain, fill out the online form above or download and complete your Table Captain Commitment Form and return it to:
